Transcript Request Guidelines:

Official transcripts will not be distributed to the student or parent. Students/parents are entitled to unofficial copies with the understanding that colleges and universities do not accept them for admissions purposes. If a transcript is collected in person, it will be stamped "unofficial."
All "official" transcripts will be mailed directly to the college, university, institution, or scholarship foundation unless otherwise directed by the school administrator or designee.
If you were a Sweetwater student who attended Wallace, your transcript requests will have to be directed to Sweetwater ISD.

Maiden Name (if applicable) What was your last name in high school?

Birthdate Enter the MONTH, DAY and YEAR that you were born. (Example: 01/01/1980 or January 1, 1980)

Telephone Number Where can we reach you if we have a question?

Graduation Year What year did you graduate? If you did not graduate enter "NONE".

School or Company Name To whom are we sending your transcript? If we are sending this to your home address, just type "home address" in this area and complete the rest of the information as requested (address, city, etc.).

School or Company Address What is the street address or post office box number of the school receiving your transcript?

School or Company City, State, Zip Code Enter the City, State, and Zip Code for the school or company receiving your transcript.

School or Company Phone Number Enter the phone number for the school receiving your transcript in case we have questions.

Email Address What is your email address? If you do not have one, enter NONE.

Other Important Information Is there anything else that you need to tell us or any other information you think we might need concerning this request?
